This form allows us to get your child medical treatment in the event of an accident or injury as well as giving your child permission to participate in any and all approved youth and church activities both on church property as well as abroad. These forms are mandatory and are required to be on file in the Youth Director's office before a youth can participate fully in the activities of the youth ministry. Please fill out this form in its entirety and return it to Matthew Johnson, Youth Director.
